Sagging Buttocks After Weight Loss: Causes and Correction Options

After major weight loss, the buttocks often lose volume and skin elasticity, here is how to address it effectively.

Significant weight loss, whether through bariatric surgery, diet and exercise, or a combination, is one of the greatest health achievements a person can make. But the body's skin does not always keep pace with the loss of underlying fat and tissue volume. The buttocks are particularly vulnerable: as the fat that gave them shape is lost, the overlying skin can sag, flatten, and hang. This is a natural consequence of significant tissue volume reduction, and it is highly treatable.

Why the Buttocks Sag After Weight Loss

The shape and projection of the buttocks depend on a combination of fat volume, gluteal muscle development, and skin elasticity. When significant fat is lost:

  • The volume that gave the buttocks fullness and projection disappears
  • The stretched skin does not retract fully, particularly after large or rapid weight loss
  • The loss of dermal elasticity with age compounds this effect
  • Gluteal muscle atrophy from reduced activity during the weight loss period may contribute

Non-Surgical Options (Limited Efficacy for Significant Ptosis)

For mild cases, non-surgical approaches may provide partial improvement:

  • Resistance training: Building gluteal muscles through squats, lunges, hip thrusts, and deadlifts can improve shape and projection, though it will not eliminate significant skin excess
  • Radiofrequency skin tightening: Devices like Morpheus8 or Thermage applied to the buttocks and posterior thighs can modestly improve skin laxity in patients with mild concerns

Surgical Solutions for Sagging Buttocks After Weight Loss

For moderate to severe buttock ptosis and skin excess after significant weight loss, surgery is typically the only effective solution:

  • Brazilian Butt Lift (BBL): For patients who lost significant fat but retain some harvestable fat elsewhere, a BBL uses fat transfer to restore lost volume and improve shape. Simultaneously, the liposuction component contours surrounding areas for a more defined result.
  • Buttock lift (gluteoplasty): Removes excess skin from the upper buttock by placing an incision in the natural gluteal crease or at the waistline. Lifts and tightens the skin without adding volume.
  • Lower body lift: For patients with skin excess around the entire lower torso, buttocks, outer thighs, hips, and abdomen, a circumferential lower body lift addresses all areas in a single extended procedure.
  • Buttock implants: Silicone gluteal implants are placed within or beneath the gluteal muscle for patients who lack donor fat for a BBL.
