Normal Pain and Discomfort After Fat Transfer

In the first few days after a BBL, patients typically experience moderate soreness, swelling, and bruising in both the donor sites (where fat was harvested via liposuction) and the buttocks. Common sensations include:

  • Aching or throbbing in the buttocks and liposuction areas
  • Tightness or firmness under the skin as transferred fat integrates
  • Tingling or numbness as nerves heal
  • Mild burning sensation along incision sites

These symptoms typically peak within the first 3 to 5 days and gradually improve over 2 to 4 weeks.

Why You Should Avoid Sitting Directly on Your Buttocks

One reason post-BBL pain can worsen is improper positioning. Sitting directly on the newly transferred fat cells compresses the delicate blood supply they need to survive. Patients are instructed to avoid sitting on their buttocks for at least 2 to 4 weeks, using a BBL pillow that transfers weight to the thighs instead. Ignoring this guideline can cause increased pain, fat cell death, and poor results.

Warning Signs That Require Medical Attention

While mild pain is expected, certain symptoms warrant immediate contact with your surgeon:

  • Sudden increase in pain after initial improvement
  • Fever above 101°F (38.3°C)
  • Excessive redness, warmth, or pus at incision sites
  • Severe swelling that worsens after the first week
  • Shortness of breath or chest pain (possible signs of blood clot or fat embolism)

Managing Pain During BBL Recovery

Your surgeon will prescribe pain medication for the first week of recovery. Beyond prescription medication, patients can manage discomfort by:

  • Wearing a compression garment as directed
  • Taking prescribed anti-inflammatories to reduce swelling
  • Staying hydrated and maintaining a nutritious diet
  • Attending all scheduled follow-up appointments
  • Beginning lymphatic massage sessions once cleared by your surgeon
