How to Sit After Buttock Augmentation

How you sit during recovery after buttock augmentation directly impacts your final results, here is the complete guide to protecting your investment.

After buttock augmentation, whether via Brazilian butt lift (fat transfer) or gluteal implants, the way you position your body during recovery is one of the most important factors in achieving your desired final result. Pressure on the buttocks in the early weeks can displace implants, compress transferred fat, reduce fat cell survival, or alter the final shape. Following the proper sitting protocol is non-negotiable for optimal outcomes.

Why You Cannot Sit Normally After a BBL

After a fat transfer BBL, the newly injected fat cells are establishing their blood supply during the first 6–8 weeks. Sitting directly on the buttocks compresses these fragile cells, restricts blood flow, and significantly reduces the percentage of fat that survives long-term. The more fat that survives, the larger and more defined your final result will be. Every time you sit directly on the buttocks without protection, you compromise the outcome you paid for.

The BBL Pillow: Your Most Important Recovery Tool

A BBL pillow (also called a booty board or post-op sitting pillow) is a donut-shaped foam or inflatable cushion that positions your weight on your thighs rather than your buttocks when sitting is necessary. You must use this pillow:

  • Any time you must sit (meals, working, driving, toileting)
  • For at least 6–8 weeks after surgery
  • Even when using a toilet, a BBL toilet riser is also available

Approved Resting Positions

  • Lying face down (prone): The preferred sleeping and resting position for the first weeks
  • Lying on your sides: Acceptable, alternating sides to avoid prolonged pressure on one area
  • Lying on your back: Not recommended until cleared by your surgeon, this applies pressure to the buttocks even when lying flat
  • Standing: Encouraged as much as comfortable, it is the most beneficial position for fat survival

When Can You Sit Normally Again?

Most surgeons clear patients for unrestricted sitting at 6–8 weeks post-operatively, once they have confirmed adequate fat survival and healing at a follow-up appointment. Some patients with larger fat transfers or slower healing may be advised to continue restricted sitting longer. Always follow your specific surgeon's instructions rather than a general timeline.
