How you care for your body after a BBL is just as important as the surgery itself, follow this guide to protect your results.

A Brazilian Butt Lift requires more active patient participation in the recovery process than almost any other cosmetic procedure. The fat cells transferred to your buttocks during surgery are fragile, their long-term survival depends directly on how well you protect them in the weeks following the procedure. Following your after care instructions is not optional; it directly determines the quality and longevity of your results.

The First 48 Hours: Immediate Post-Operative Care

The first two days after your BBL are the most critical for initial stabilization:

  • You will leave the surgical facility in a compression garment covering the liposuction donor sites, wear it continuously
  • Do not sit or lie directly on your buttocks, the BBL pillow or lying face-down must be your only positions
  • Take all prescribed medications as directed, including antibiotics and pain management
  • Stay well hydrated, aim for eight or more glasses of water
  • Have a trusted adult caregiver present at all times during the first 48 hours
  • Avoid strenuous activity of any kind

Weeks 1–6: Protecting the Fat Transfer

The first six weeks are when fat cell survival is established. The transferred cells are integrating with the surrounding tissue and developing new blood supply:

  • No direct buttock pressure: Use a BBL donut pillow whenever sitting is unavoidable. Position weight through the thighs, not the buttocks. This is the single most important instruction.
  • Sleeping position: Sleep face-down or on your side. Back sleeping compresses the fat grafts and reduces survival.
  • Compression garment: Wear your provided garment over the liposuction sites continuously for the first four to six weeks. Do not apply compression directly to the buttocks unless instructed by your surgeon.
  • Light walking: Begin short, slow walks as early as day two to promote circulation and reduce the risk of blood clots. Gradually increase duration as healing progresses.
  • No vigorous exercise: Avoid running, cycling, squats, or any exercise that engages the gluteal muscles or involves sitting on exercise equipment.

Nutrition and Hydration for Fat Survival

What you eat and drink during recovery directly supports the viability of your transferred fat cells:

  • Eat a protein-rich diet (60–100 grams daily) to support tissue repair and fat cell integration
  • Stay hydrated, dehydration impairs lymphatic drainage and healing
  • Avoid alcohol for at least three to four weeks, it impairs healing and interacts with medications
  • Do not attempt aggressive caloric restriction during recovery, the transferred fat cells need adequate nutrition

Months 2–3: Transitioning Back to Normal Activity

Once your surgeon clears you at the six to eight week mark, you can gradually resume normal activities:

  • Begin sitting normally without the BBL pillow when your surgeon confirms fat integration is complete
  • Reintroduce exercise gradually, starting with low-impact activities before heavy weight training
  • Continue lymphatic massage sessions through month three for optimal contouring
  • Attend all scheduled follow-up appointments to monitor your healing progress
