The BBL has historically carried one of the highest mortality rates among elective plastic surgeries, primarily due to fat embolism risk when fat is injected too deeply. Higher BMI increases multiple risk factors:

  • Longer surgery time increases anesthesia risk and blood clot formation risk
  • Excess fat in donor areas can make it harder to achieve precise, natural-looking results
  • Obesity is associated with impaired wound healing and higher infection rates
  • Significant post-operative weight changes can alter or compromise BBL results

Can I Get a BBL If My BMI Is Over 35?

Some surgeons will perform BBL on patients with a BMI up to 35–37 with appropriate risk counseling and screening. However, most reputable, safety-focused practices set a hard limit at 35 for new patients. If your BMI is above this threshold, your surgeon will typically recommend weight loss before scheduling surgery. A 10–15 lb reduction can make a significant difference in your risk profile.

What If My BMI Is Too Low?

For very lean patients, a traditional BBL may not be feasible due to insufficient donor fat. Options include:

  • Skinny BBL: Micro fat grafting using small volumes from multiple donor sites
  • Butt implants: Silicone gluteal implants for patients who cannot produce enough transfer fat
  • Combination approach: Small-volume fat transfer plus implants for enhanced shape and volume
