BBL Surgery Requirements: Who Qualifies for a Brazilian Butt Lift?

A Brazilian Butt Lift is one of the fastest-growing cosmetic procedures in the United States, but it is not appropriate for every patient. Because the procedure involves both liposuction and fat transfer, it carries unique requirements related to body composition, health status, and realistic expectations. Body Composition Requirements

The most fundamental requirement for a BBL is having sufficient donor fat, extra fat that can be harvested via liposuction and transferred to the buttocks:

  • Adequate fat stores: You must have enough fat in donor areas (abdomen, flanks, back, thighs, or arms) to harvest a meaningful volume. Patients who are very lean or have a very low BMI may not have enough transferable fat for a significant result.
  • BMI range: Most surgeons prefer patients with a BMI between 22 and 32. Patients with a BMI below 20 may lack sufficient donor fat; those above 35 face elevated surgical and anesthesia risks.
  • Weight stability: Your weight should have been stable for at least three to six months before surgery. Significant weight loss or gain after a BBL can unpredictably alter your results.

Medical Health Requirements

  • Non-smoker or smoking cessation: Smoking impairs wound healing and fat cell survival. Most surgeons require complete cessation for at least four to six weeks before and after surgery.
  • No active infections or skin conditions: Any skin infection, open wound, or active skin condition in the surgical area must be fully resolved before surgery.
  • Controlled chronic conditions: Diabetes, hypertension, and other chronic conditions must be well-managed. Uncontrolled metabolic conditions significantly elevate surgical risk.
  • No active blood clotting disorders: These must be identified and discussed with your surgical team, as BBL involves risks related to fat embolism.
  • Age requirement: Patients must be at least 18 years old. Most surgeons prefer candidates to be 21 or older when the body has fully developed.

Psychological and Expectation Requirements

  • Realistic expectations about what a BBL can achieve, dramatic results require adequate donor fat
  • Willingness to follow post-operative instructions, including no sitting on the buttocks for six to eight weeks
  • No history of body dysmorphic disorder (BDD) without psychological clearance
  • Surgery motivated by personal goals, not external pressure
