BBL Revision: When You Need It and What to Expect

A revision BBL can correct asymmetry, volume loss, or unsatisfactory shaping from a previous procedure.

Not every BBL delivers the result a patient hoped for. Whether due to excessive fat reabsorption, asymmetry, uneven contours, or simply not achieving the desired volume or shape, a BBL revision can address these concerns. Here is everything you need to know about the revision process.

Common Reasons for BBL Revision

Patients seeking BBL revision most commonly present with one or more of the following:

  • Volume loss: More fat reabsorbed than expected, leaving the buttocks smaller than desired
  • Asymmetry: One side appears significantly different in size or shape from the other
  • Flat spots or dents: Irregular contours due to uneven fat survival
  • Insufficient waist definition: The donor site liposuction did not create the hourglass effect desired
  • Sagging over time: Natural tissue changes following significant weight loss or pregnancy after the original BBL
  • Desire for more volume: Patient is happy with shape but wants additional augmentation

When Can You Have a BBL Revision?

Most surgeons will not evaluate a patient for BBL revision until at least 6 months after the original procedure. This allows all swelling to resolve and results to fully stabilize. Attempting revision too early risks operating on tissue that is still healing, increasing complication risk and producing unpredictable results. For patients whose first BBL was performed elsewhere, bringing operative records and photos is helpful for the revision consultation.

What Does BBL Revision Involve?

The revision process is similar to the original BBL but adapted to address specific concerns:

  • Additional liposuction from available donor areas to harvest new fat for transfer
  • Re-injection of purified fat into deficient areas or to increase overall volume
  • Corrective liposuction to refine waist and hip contours if needed
  • In cases with very limited donor fat, combination with gluteal implants may be considered

Candidacy Requirements for BBL Revision

The same BMI and health requirements that apply to a primary BBL apply to revision surgery. Key additional considerations include:

  • Sufficient donor fat must be available for meaningful transfer
  • The original BBL must be fully healed (minimum 6 months)
  • No active infections or unresolved complications from the prior surgery
  • Realistic expectations about what revision can and cannot achieve
